LIVORA NUTRITION
COACHING AGREEMENT & LIABILITY WAIVER
Coach: Livora Nutrition LLC
This Agreement outlines the terms of nutrition and lifestyle coaching services provided by Livora Nutrition LLC. By signing or replying via email in agreement, the Client agrees to the following:
Scope of Services

Coaching includes personalized nutrition and lifestyle guidance, goal setting, and accountability.

Coach is not a licensed medical or mental health professional. Services do not replace medical advice or therapy.

Client is responsible for their own decisions, actions, and outcomes.
Communication & Boundaries

Check-ins are scheduled weekly. If missed, Coach will respond using available information.

Communication outside of check-ins may require a scheduled session.

Coaching is professional and respectful. Coach maintains an objective, supportive stance.
Financial Terms

Client agrees to a 3-month coaching commitment.

Payment is due prior to the start of each month.

After 3 months, client may cancel with 30 days written notice.
Termination

Coach may refer the Client elsewhere if coaching is no longer appropriate or effective.
Waiver & Release of Liability
Client acknowledges risks associated with physical activity and lifestyle change. By entering into this agreement, the Client affirms they are physically and mentally capable of participation and agree to assume all risks. Client releases and holds harmless Livora Nutrition LLC, its employees, agents, and affiliates from any and all claims, liabilities, or damages arising from services or participation.
Photo & Media Release
Client grants permission for Livora Nutrition LLC to use photos or videos for marketing purposes, only with prior approval.
Legal
This Agreement is governed by Ohio law. Any disputes shall be resolved in Butler, Ohio. If any part of this Agreement is found unenforceable, the remainder shall remain valid.
By checking this box in agreement and completing a transaction, the Client agrees to all terms, including this contract, waiver, and release of liability.
